Rural Health Transformation Program
SMG can support CAHs and rural hospitals in practical, high-impact ways that align with the goals of the Rural Health Transformation Program.
What is the Rural Health Transformation Program?
The Rural Health Transformation Program is a $50 billion federal initiative running from 2026 through 2030 designed to strengthen rural healthcare systems, including Critical Access Hospitals. Funding is performance based, tying dollars directly to measurable outcomes, with first year awards averaging $200 million per state.
States are actively defining priorities and performance metrics now. Early engagement positions rural hospitals to secure funding and drive sustainable improvements in access to care, operational efficiency, and long term financial stability. SMG can support CAHs and rural hospitals in practical, high-impact ways that align with the goals of the Rural Health Transformation Program:
-
Close critical gaps in care delivery by strengthening local access points and reducing the need for patients to travel long distances. Focus areas include improving primary care availability, expanding specialty partnerships, enhancing emergency response capabilities, and building stronger chronic disease management programs.SMG put an aggressive turnaround plan into place. Patient rooms were renovated, therapy services were expanded and strategic decisions were made about the facility’s service lines. Today, the facility has an 18-bed med/surg unit, a 98-bed extended care facility, three provider-based Rural Health Clinics, and a state of the art gym and wellness center.
-
Stabilize operations by improving margins, optimizing reimbursement, and aligning services with community needs. The goal is to create financially viable rural providers that can serve as dependable access points for years to come.
-
Address workforce shortages through targeted recruitment strategies, retention incentives, leadership development, and rural focused training programs that build a stable and committed clinical team.
-
Shift from volume driven care to outcome focused models that reward quality and efficiency. This includes developing regional partnerships, care collaboratives, and flexible delivery models that improve performance while controlling costs.
-
Leverage telehealth, remote monitoring, advanced data systems, and optimized EHR platforms to increase access, improve care coordination, and enhance operational efficiency across rural settings.
This program is built to address the most pressing challenges facing rural providers, including tight margins, workforce shortages, limited specialty access, and growing chronic disease burdens. Its core objectives include:
